Congratulations on your acceptance to the 2025 Atlanta Horn Festival!

Please complete the following digital forms* in addition to your tuition payment by May 1, 2025 to secure your position.

*By completing the below forms, you consent to the use of digital signatures. High School Participants should complete the below forms with the assistance of their Parent/Legal Guardian.

1. Code of Conduct & Media Release

The Atlanta Horn Festival has established rules and standards of conduct for all Participants. It is the responsibility of the Participant and the Parent/Legal Guardian to review the Program rules and standards of conduct. If applicable, dismissed Participants are not eligible for a refund of any fees or expenses. The Participant or Parent/Legal Guardian is responsible for all costs associated with removing the Participant from the Program due to misconduct, including but not limited to transportation costs to return the Participant home.

Participant Agreement 

I understand that as a condition for participating in the Atlanta Horn Festival I must comply with the Festival’s rules and standards of conduct and follow all reasonable directions of the Festival Staff. Failure to comply with the Festival’s rules and standards of conduct or failure to comply with the reasonable direction of Festival Staff may result in my being dismissed from the Festival.

Participants agree to:

  • Attend all applicable festival activities and treat our host location with care and respect.

  • Treat all participants with respect & maintain an encouraging, supportive learning environment.

  • Dress in an appropriate manner that is not revealing or distracting to others. No see-through clothing, exposed midriffs, or excessively revealing clothing is allowed.

  • Bring all necessary music, writing utensils, and items for playing & maintenance daily.

  • Arrive with basic familiarity of horn choir part assignments. No sight reading is allowed.

Transportation

The Atlanta Horn Festival does not provide transportation to or from the event and assumes no liability for student travel.

  • Young Artist Division participants 18 and over may leave campus for lunch but must be warmed-up (if required) and on time for the post-lunch activity.

  • High School Division participants may not leave campus until the conclusion of their respective program’s activities.

Media Release

I acknowledge that persons employed by the Atlanta Horn Festival may take photographs and/or videos of my participation and allow the use of these materials on behalf of the Event without limitation or compensation, including the release of my name. If you prefer not to appear in photographs/videos, please indicate so when completing this form.

Tuition

Tuition payments are non-refundable. If a student must cancel participation in the Atlanta Horn Festival, they may defer their acceptance to the following year and apply their tuition payment toward the following year’s tuition. Unused tuition payment funds shall be forfeited if not applied by the season following the original acceptance offer.

3. Emergency Information

Administration of Medication 

Atlanta Horn Festival faculty, staff and volunteers are not equipped to administer medications to Participants. All Participants should be able to administer their own medications.

Authorization for Medical Treatment 

I consent to medical and/or surgical care as may become necessary for the Participant’s well-being, should the need arise, and I understand that I will be solely responsible for the cost. I authorize the Atlanta Horn Festival to communicate in emergencies with the person(s) identified in my submission materials. I hold harmless and agree to indemnify the Atlanta Horn Festival & Morningside Presbyterian Church from any claims, causes of action, damages, and/or liabilities arising out of or resulting from said medical treatment.

By signing the below forms, I agree that all information is accurate and current, that all important information is listed, and to the best of my knowledge, I am/my child is capable of participating safely in the Atlanta Horn Festival. I acknowledge that my failure to disclose relevant information may result in harm to myself, my child, and/or others during this program. I agree to notify the program of any changes in the above information as soon as possible.

4. Parent/Legal Guardian Agreement for Minors
(Only required for Participants 17 and under)

I understand that my child will be subject to the rules and standards of conduct of the Atlanta Horn Festival. I further understand that my child’s violation of the rules and standards of conduct or failure to comply with the reasonable direction of Festival Staff may result in my child’s dismissal from the Festival. I accept responsibility for all costs associated with removing my child from the Program, including but not limited to transportation costs to return the Participant home. I understand that dismissed Participants are not eligible for a refund of any fees or expenses.
